Job Description

  • You may be required to attend multi agency meetings, CAF’s and Annual Reviews
  • Support on the completion of Education, Health and Care Plans (EHCPs) and the securing of higher needs funding to meet identified needs
  • Work with senior colleagues to ensure that the service meets its responsibilities under the Equalities Act (2010) with regard to reasonable adjustments and access arrangements
  • Ensure that service policies, procedures and practices relating to SEN are adhered to
  • In partnership with senior colleagues, monitor and evaluate the quality of teaching and learning
  • Help colleagues to evaluate the impact of their teaching on learners.
  • Ensure continuity and progression through the analysis of learner performance data
  • Liaise with external agencies, commissioners and other stake holders to communicate strategy and progress in relation to SEN particularly in preparation for Ofsted
  • Ensure that the service keeps the records of all learners with SEN up-to-date
  • Liaise with the designated safeguarding lead where there are issues relating to a learner with SEN
  • Advise on the deployment of the service’s delegated budget and other resources to meet learners’ needs effectively.

Recording and Assessment

  • Work with colleagues to identify and set challenging targets for raising achievement among pupils with SEN
  • Ensure that robust tracking systems are in place to collect and interpret specific pupil level assessment data, allowing the service to identify ‘value added’ resulting from teaching and intervention strategies
  • Manage systems for screening learners at ‘point of entry’, identifying, assessing and reviewing provision
  • Attend review meetings, parent consultations and other meetings relating to provision for SEN when required
  • To undertake any other duties that can be reasonably expected of you within your job grade

The role of Case Manager is an excellent opportunity to deliver a range of holistic and supportive employability interventions to refugees, as part of a new Home-Office funded contract. Reporting to the Operations Manager, and working alongside a large team, this role is key to providing timely, supportive, and sustainable impact for participants.

About You

  • Approachable, adaptable, and supportive to the range of stakeholders involved in this programme.
  • Strong interpersonal and communicative skills, with the ability to develop strong rapport with referral partners, customers, training providers, employers, and participants.
  • Experience of providing employability-related coaching support, to build the self-confidence, skills, and abilities of job seekers/learners.
  • Great organisational, administrative, and prioritisation skills, with the ability to work to deadlines, produce reports, and deliver on objectives.

Main Duties and Accountabilities

  • Inducting, assessing, and 'on-boarding' participants, delivering high quality end to end information, advice, and guidance.
  • Promoting and developing the mind-set, emotional intelligence, self-confidence, and esteem of participants, so that they are better equipped, and succeed in the world of work.
  • Preparing CVs, cover letters, and job/apprenticeship application forms with participants, and undertaking job searches using relevant web-based software and packages.
  • Short listing, pre-selecting, and matching/submitting participants to vacancies/work experience opportunities generated by the Catch22 account management team, and preparing job packs on specific vacancies/work experience opportunities.
  • Introducing participants to a 'strength based' recruitment approach to succeeding in job interviews, preparing them to a high standard for interview with employers, and arranging interviews for participants where appropriate.
  • Engaging participants to ensure that they sustain their job or apprenticeship, and delivering high quality In Work Support, which effectively sustains participants for a minimum of six months.
  • Supporting with the delivery of any Job Fairs, Workshops, and Seminars, both face to face, and remote, that prepare participants ready for the world of work.

The role of Case Worker - County Lines is an excellent opportunity to provide one-to-one tailored support to young people, working collaboratively as part of a multi-agency team, to ensure the safety of young people, and maximise their outcomes. Tailoring support to each young person's individual needs and circumstances, our case workers work as part of the Home Office-funded County Lines Support and Rescue Service.

About You

  • Great verbal and written communication skills, with the ability to maintain relationships with key partner agencies, working effectively as part of a team.
  • Honest and reliable, with a creative and flexible approach to all aspects of work.
  • Strong organisational skills, with the ability to manage conflicting demands through a problem solving and a ‘can do’ attitude.
  • Experience of working with, and delivering training to, young people and professionals, in both group and one to one settings.
  • Strong attention to detail, and knowledge of risk assessments, engagement strategies, intervention strategies, particularly in relation to young people with complex needs.
  • Experience of using recording and data collation systems.

Main Duties and Accountabilities

  • Providing appropriate end to end support, and interventions, to a pre-defined caseload of young people, and their families, who are involved in County Lines activity – young people are referred by means of effective assessment, care planning, and risk management.
  • Proactively and continually assessing and managing caseload risks, escalating where necessary through internal and external safeguarding procedures, and adapting case management frequency and methodology to reflect individual needs and circumstances.
  • Ensuring that the diverse needs and aspirations of all groups of service users are considered in service planning and delivery, to enable all service users to fulfil their potential.
  • Liaising effectively with local partner organisations, and other relevant agencies, to ensure effective referrals, care, risk management, and ease of transition and referral, for young people.
  • Promoting the continued joint work of the service, and other partners, acting as a sub-regional or district single point of contact, where required.

Qualifications

  • Experience of working with young people (18-25) and their families, identified as vulnerable or having complex needs, preferably in a community setting.
  • Experience of inter-agency working, preferably with Children's and Family Services/Youth Justice Services/Youth Services.

Qualifications

  • A recognised teaching qualification (L3 minimum).
