How will this role have an impact?

This position is responsible for providing technical and operational support to Signify Health’s Clinician Network (Providers) and Signify Health’s employee groups. Our clinician network includes healthcare professionals (Nurse Practitioners, Doctors, etc) that provide home healthcare services. Providers leverage our Signify Health mobile application to host medical screenings, assign and regulate medications, and schedule and capture patient information during appointments. You will be the First Point of Contact for our external providers and our internal Signify Health employees.

This role requires a customer first mindset when interacting with all end users.

This role will report to our Service Desk Supervisor

What will you do?

  • Day to day technical and application support for external clinician network and internal Signify Health employees
  • Customer engagement via ticketing system, inbound and outbound, and email channels
  • Resolve iOS and Signify Application related issues.
  • Resolve basic ADP, OKTA and other application issues. Basic MacOS, Windows OS devices for external provider Signify Health employees
  • Respond, coordinate, and monitor hardware fulfillment requests for iPad and peripherals
  • Escalate and manage tickets transferred to other departments
  • Keep customers and tickets documentation up to date
  • Respond to customer questions and as needed guide customer to resolution
  • Follow up with customers on unresolved issues for reopened tickets and callbacks
  • Actively demonstrates teamwork at all times by collaborating on ad hoc projects
  • Ability to work occasional overtime, weekend, and holidays

We are looking for someone with:

  • High School Diploma or equivalent.
  • Minimum 1 to 2 years of successful work with technical support in a helpdesk environment
  • ‘Customer First’ Mindset and Collaborative
  • Experience supporting Windows OS / Hardware, Apple iOS/ Software / iPad hardware via phone and email
  • Proven analytical and problem-solving abilities
  • Experience with Helpdesk ticketing systems
  • Strong customer service skills (Written and verbal)
  • Experience with web-based applications and mobile device management tools (Google Workspace, Slack, MaaS360, Workspace One) preferred

The base salary hiring range for this position is $16.00 to $28.00. Compensation offered will be determined by factors such as location, level, job-related knowledge, skills, and experience. Certain roles may be eligible for incentive compensation, equity, and benefits.

Qualifications

Responsibilities will include:

  • Conduct a hybrid of in-home and telehealth video visits for Devoted Health members within your patient panel and who are assigned to your geography. In infrequent instances when appropriate and compliant with NP licensure, you may also provide telehealth visits to members located in other geographies in order to support other Devoted Medical service lines. 

  • Perform comprehensive assessment visits (CAV), comprehensive diagnosis/disease review, medication review, and assessment for quality of care (Stars/Hedis) interventions as well as social and home health/DME needs as a tool to identify patients eligible for IHC enrollment.

  • Develop and provide plans of care in collaboration with IHC team members to manage complex patient populations. 

  • Perform appropriate diagnostic tests and interventions. 

  • Meet patients’ needs through a variety of visit types depending on their need, including chronic care, urgent care, post hospital transition of care, and follow up visits, with collaboration and support from the IHC team. 

  • Work closely with other members of the patient's care team including their PCP, specialists, and other Devoted teams such as  pharmacy, or specialty care.  You will also collaborate with the patients’ support systems such as family members and caregivers in order to coordinate care for the patient and deliver a collaborative care plan.

  • In certain geographies, there may be an infrequent weekend on-call component to support our clinical nurses who triage calls from our members during the weekend.

    What You'll Do:

    • Deploy, automate, manage, and maintain the system.
    • Ensure the system's availability, performance, security, and scalability.
    • Manage the creation, release, and configuration of the system.
    • Evaluate new technology alternatives and vendor products.
    • Troubleshoot and resolve issues across various application domains and platforms.
    • Ensure critical system security by leveraging best practices and robust cloud security solutions.
    • Provide recommendations for architecture and process improvements.
    • Define and deploy systems for metrics, logging, and monitoring on a cloud platform.
    • Design, maintain, and manage tools for automating different operational processes.

    What You'll Bring:

    • Bachelor’s degree in computer science or similar field or equivalent experience
    • 3+ years of work experience as an Infrastructure / DevOps engineer in a professional full-time role
    • Over 3 years of experience in provisioning, operations, and management of cloud-native environments
    • Proficiency with Git and GitHub workflows
    • Strong knowledge of Ruby, Python, or other scripting languages
    • Experience with cloud native infrastructure (Azure preferred)
    • Experience with Container Applications (Docker)
    • Experience with infrastructure as code tools like Terraform or Azure Resource Manager (ARM) templates
    • Ability to work independently
    • Strong problem-solving and critical thinking skills
    • Excellent communication and collaboration skills to work effectively with cross-functional teams

    Set Yourself Apart:

    • Professional experience in application programming with an object-oriented language
    • Experience with multi-tier architectures: load balancers, caching, web servers, application servers, databases, and networking
    • Comprehensive knowledge regarding contemporary processes and methodologies for development and operations

    Expected Salary Range: $115,000 - $140,000 base + bonus

    This position can be remote in the United States or hybrid in Chicago, IL or Boston, MA. Expected hours will be Eastern or Central time.

    Job Description

    Sales Development Coordinator plays a critical role in serving Sseko’s mission to use business to create educational and economic opportunities for women across the globe.  This role will support our sales and field development team across all sales and development functions and activities. 

    This role requires an action-oriented, detail focused, team player who can create clear and accurate reporting, manage details across departments and support the sales team in driving community engagement.  

    Key Responsibilities: 

    • Manage, own and distribute all reporting needed to support sales team 
    • Support Events, Team Calls and other recognition events with deck building, notes, scripts, facilitating, follow-ups and needed reporting.
    • Manage and own production of Fellows virtual experiences. Run point on production for virtual events, monthly calls or virtual training experiences
    • Support and participate in semi-annual field conferences and events. 
    • Manage inbound Fellow leads.
    • Manage logistics and calendars for all trainings and events for internal team and the field.
    • Create and manage Fellows feedback loops (ie Launches, Fresh Fellows, Retired Fellows etc)
    • Check for details, accuracy and consistency across all sales field communication channels.
    • Manage video content and needed edits for video training (working with freelance video editors or in-house resources)

    Qualifications

    • Direct sales experience preferred
    • Ability to work in a fast paced, in-the-moment environment
    • Excellent communication skills with obsessive attention to detail 
    • Highly organized with the skill to track projects from inception to final delivery 
    • Capacity to manage multiple priorities and deadlines and work quickly and efficiently in a fast-paced environment 
    • Proficient in Excel, Google Sheets and other reporting and analytics tools 
    • Proficient with Facebook groups, events, pages, and other FB tools
    • Proficient writer: clear, concise, fun! 
    • Adaptable!  It’s a fast moving, fluid environment, and you must be able to wear multiple hats when needed
    • Comfortable in Canva with basic design tools
    • Proficient with Google Suite, Asana and Slack
    • Self-starter, fast learner with action and results-orientation
    •  Resourceful, ability to problem-solve and react quickly with a positive can-do attitude

    The ideal teammate would be able to:

    • Conduct behavioral health initiating visits for members with behavioral or psychiatric conditions that warrant Behavioral Health Integration (BHI) services
    • Review care plans conducted by our clinical staff, focusing on behavioral health counseling and education 
    • Provide routine psychiatric services to a panel of patients, reviewing their clinical status and providing recommendations for their care
    • Responsible for conducting cognitive assessments for suspected cognitively impaired populations, ensuring comprehensive care for all patients
    • Conduct care coordination and recommend/identify cost effective research based treatment and intervention
    • Utilize strong clinical skills in physical assessment and chronic disease management for at risk adults and apply member specific Care Management and individualized care planning
    • Be comfortable with advanced care planning discussions with caregivers and members
    • Attend meetings, training sessions and participates on committees as needed
    • Possess a strong knowledge of clinical procedures, standards and quality control checks
    • Possess a strong knowledge of medical conditions, interventions and treatment
    • Provide members, caregivers and facility education
    • Monitor the quality of member’s care and updates plan of care

    Would you describe yourself as someone who has:

    • Certified and licensed as a Nurse Practitioner in good standing (required) in the state of New York (preferred)
    • Bilingual Fluency in Spanish and English (required)
    • Certification from ANCC (or equivalent) as a Psychiatric Mental Health Nurse Practitioner AND Primary Care, Family, Adult/Geriatric or Acute Care Nurse Practitioner (required)
    • Experience in performing cognitive assessments, as well as administering tools such as PHQ2s, PHQ9s, and GAD-2 assessments (required)
    • Master’s or doctoral degree from an accredited institution for nurse practitioners (required)
    • Medicare participation and ability to have the company bill for services on your behalf (required)
    • 1+ years of Nurse Practitioner Experience (required), qualified  for independent practice in your licensed jurisdiction (preferred
    • 2+ years of clinical experience working with complex adult populations (required)
    • Ability to practice independently with little clinical support (required)
    • The ability to work remotely and has a private area with a computer in their home/workspace (required)
    • Comfort using technology like Google Suite, multiple EMRs, Slack (required)
    • Experience working in home care and/or family medicine, geriatrics (preferred)
    • Experience in prescribing and medication reconciliation for psychiatric and behavioral health patient populations. 
    • Experience working within a clinical team environment
    • Strong organizational skills, including the ability to prioritize
